Procedures and Forms for Requesting Assistive Technology Support
Student Specific Consultation Request:
When considering the need for assistive technology consultation for a student, the classroom teacher and/or the Special Education Team members are required to fill out the Instructional Support Consultation Request form. Complete the consultation request form below and email the completed form to the School Psychologist and to the Assistive Technology Specialist assigned to your school. Please also keep a copy for your records. Based on this process, the Assistive Technology Specialist will determine whether a referral for a full evaluation is appropriate.

Classroom Consultation Request:
When requesting assistive technology consultation around the use of low to high technology solutions for a group of students and/or all the students in the class the classroom teacher and/or a Special Education Team are required to fill out the Classroom Assistive Technology Consultation Request. Complete the consultation form below and email the completed form to the School Psychologist, the Assistive Technology Specialist assigned to your school, and keep a copy for your records
