Open Now: Submit your student’s vaccination info!

CPS is requiring all students ages 12+ to be fully vaccinated in order to participate in CPS-sponsored before and afterschool activities, including athletics, clubs, visual and performing arts programs, and school-sponsored social events.

Families can submit information via the Family Portal. Students ages 18+ may submit their information through a special student link via the Family Portal.

Families may request exemptions for medical or religious reasons or for students with special needs. Please be patient as we work together to collect and review student vaccination information. Vaccination deadlines are different for different activities.

Winter Athletic Registration Requirements

Winter Sports Online Athletic Registration Deadline: November 21st

1. Vaccinations - All students must be fully vaccinated to play a Winter Sport.

If you have been vaccinated and have not uploaded your vaccination status, or exemption status, you can do so at the CPS Vaccination Portal. Please be sure to upload this right away so that your upload can be reviewed and approved.

If you have not received any of the vaccination shots information for vaccinations can be found at the link below. Remember that being fully vaccinated is 14 days after your final dose. This means that for Pfizer or Moderna you get one shot, 14 days later you get a second shot and 14 days later you are now fully vaccinated.
