Annual Student Forms: 3 Options
Every year, CPS asks families to update their permissions and preferences through our Annual Student Forms process. With the launch of our new Annual Forms Gateway, the paper version of this mailing will arrive a little later than usual. However, families should receive their packet before Labor Day Weekend.
There are three ways to complete this important paperwork, if you haven't already:
- Complete the paper packet and send it to school with your child on their first day.
- Look for your child's Student ID# and log into the system using the instructions in the packet.
- Print a complete set of forms from our website and send it to school on the first day.
