Key Information for In-Person Learning

TestingSign Student COVID-19 testing Consent Form
CPS and the Cambridge Public Health Department are partnering to provide a comprehensive student COVID-19 testing plan. This plan includes both surveillance testing and pooled testing (to detect COVID-19 cases among individuals who are not displaying symptoms) and symptomatic testing (when students display symptoms during the school day).

COVID19 Student Testing

We cannot require students to participate in testing. However, we strongly encourage students to participate in this opportunity to help protect the health of your student, your family, and our school community.

Sign consent forms
Families must provide permission for students to participate in testing. Log-in to the Caregiver portal to sign the forms:
Elementary students: two forms

  • Pooled testing
  • Symptomatic Testing

Upper and high school students: one form for both surveillance and symptomatic testing

Remote LearningRequest a switch from remote to in-person learning:
CPS families/ caregivers may request a change to a student’s instruction mode (in-person or remote learning) through the COVID-19 Hardship Appeal process.

Request: Submit the request form through the Caregiver portal to your principal.

Notification: Schools will review the request and notify families of decisions as soon as possible, within 7 business days. The principal may follow-up to request additional information.

Transition: Any changes will be made as soon as possible, within 6 weeks (per state guidelines).
