Online Request to Transfer Form: Grades Preschool-5

The form below is for families of CPS students who will be in grades Preschool-5 to request school transfers for the next school year starting in September 2026. If your child was not on a waitlist as of December 22, 2023, you may choose one school for a possible transfer.

All renewals for the next school year must be submitted between October 15 and November 15.

Once submitted, a confirmation email will be sent to your submitted e-mail address within three to five business days. If you do not receive a confirmation email, please contact us in order to confirm that we have received your online submission.

Other Options to Request to Transfer to One CPS School
If you wish for additional options to request a transfer for the next school year, you can do the following:

• Email Kathy Sampson directly with your full name, your child's full name, current school, and current & next year's grade, child's date of birth, and the school to which you want your child to possibly transfer.
• Download and fill out the Request to Transfer Form and check the "new request" box next to the school and submit it to the Student Registration Center through scanned e-mail or fax to 617.349.6552.
